Can't edit email for download form appearance

Hi,
I just created an email-for-download form/campaign (was redirected from MailChimp) to use on my Tumblr-based site, and though I did get an embed code and it seems to work properly (the download goes through), I haven't been able to edit it's appearance at all. None of the editing done on the MailChimp site seems to be relevant. The main issue I have is that there are no fields for subscribers to add their name and surname, and I would like to have that option.
Could you please tell me if there's a way I can fix this?
(the interface in the short tutorial video I found on MailChimp is completely different from what it looks like now, so I haven't been able to use that as a reference to address the issue).
I have to send off this campaign by tomorrow, so any help asap would be great.
